What is an Immediate Load Implant?
An immediate-load dental implant is one restored with a provisional fixed crown or bridge on the same day it is surgically placed. The provisional stays in position during the 3 to 4 month osseointegration period. The definitive final crown or bridge is fitted once the implant has fully fused with bone. The protocol exists for two reasons: aesthetics (no visible gap, no removable temporary), and convenience, particularly for international patients combining surgery with travel. Important: immediate loading is not the same as immediate placement, the first means a same-day crown, the second means an implant placed into a fresh extraction socket. They can be combined when conditions allow.

Immediate loading is only safe when the implant achieves enough primary stability at placement to withstand light chewing forces during integration. CBCT bone density, intra-operative insertion torque and stability quotient (ISQ) are all measured. Below threshold, we delay loading and use a removable provisional. The implant outcome is identical, you just don't get same-day fixed teeth. We say so honestly rather than risk an early failure.

Candidacy
Are You a Candidate for Immediate Loading?
Immediate loading suits specific patients and specific cases, not everyone. The CBCT scan and intra-operative measurements decide it. Here's the honest version, before any deposit is taken.
You're a strong candidate if
- Your CBCT shows dense cortical bone at the implant site (lower jaw, mandible, often best).
- The implant achieves insertion torque of ≥35 Ncm (single) or ≥45 Ncm (All-on-4) at placement.
- Your stability quotient (ISQ) reads ≥70 on the resonance frequency device at placement.
- You have light to moderate bite forces, not severe bruxism.
- The implant position is anterior or favourable posterior, not heavy molar load on poor bone.
- You're willing to follow the 6 to 8 week soft-diet protocol after surgery.
Immediate loading may not be right if
- Your CBCT shows soft cancellous bone (often upper back jaw) where primary stability would be marginal.
- Insertion torque or ISQ measurements at placement fall below threshold (we delay loading rather than risk it).
- You have severe untreated bruxism and won't wear a night guard.
- You smoke heavily and aren't willing to stop during the 6 to 8 week healing window.
- You have uncontrolled diabetes (HbA1c above 7%) compromising integration.
- The implant position is a heavy-load posterior molar with marginal bone.
If immediate loading isn't safe for your case, we use a removable or fixed bonded provisional during integration and load the final crown at 4 months. The implant outcome is identical, only the in-between aesthetics differ.

Caring for Your Same-Day Implant
The first 8 weeks are critical. The implant is integrating, and the provisional must not be overloaded. Here's the cadence that protects the result.
Days 1 to 7
Soft Food, No Smoking
Soft food only. No smoking, no straws, no vigorous rinsing. Mild swelling is normal and peaks around day 3. Take prescribed antibiotics and pain relief as directed.
Weeks 1 to 8
Soft Diet Phase
Cooked vegetables, eggs, fish, soft pasta, soft fruits. Avoid: apples, nuts, hard bread crusts, chewy steak, ice. The implant must not take heavy chewing load while integrating.
Months 3 to 4
Final Restoration
X-ray confirms integration. iTero impression taken, definitive crown or bridge fabricated and fitted. After this point, eat normally. Brush and floss as you would any tooth.
Every 6 to 12 Months
Recall & X-Ray
Twice-yearly hygiene visits and a 12-month X-ray to confirm bone levels are stable around the implant. Standard practice for all implant patients globally.
What to avoid during healing
Smoking is the single biggest threat to immediate-load success. Untreated grinding (bruxism) is the second, if you grind, we provide a night guard during healing. Both compromise the integration window in ways that delayed-load implants tolerate but immediate-load implants do not.
If you travel home mid-treatment
You leave with complete clinical records, X-rays, implant serial numbers and surgical notes. Your home dentist can perform recalls or address concerns, and we respond to questions over WhatsApp at any time during the integration period.

Immediate loading is well-documented but not risk-free, and the risk profile differs from delayed-load implants. Here's what the literature reports and how Picasso minimises every avoidable risk.
Documented Success Rate
Long-term clinical literature reports immediate-load implant survival in the 94 to 97% range over 10 years for properly selected patients, slightly below the 95 to 98% range for delayed-load implants but still in the high-90s. Patient selection matters more than the technique itself.
What Can Go Wrong
Failure to integrate from premature loading (rare with proper torque thresholds). Provisional fracture (replaceable in the chair). Soft-tissue recession around the provisional in thin biotypes. Peri-implantitis if hygiene is poor during healing.

Smoking and uncontrolled grinding are the two biggest threats during the immediate-load healing window. Honest disclosure during consultation lets us adjust the protocol, sometimes recommending delayed loading or a night guard during integration.
Soft-Tissue Aesthetics
Even a perfectly placed immediate-load anterior implant can show subtle gum recession in thin biotypes. We assess gum biotype before surgery and may recommend a connective tissue graft to thicken the gum, predictable, gentle, done under local.

Is immediate loading the same as immediate placement?
No, they are different concepts. Immediate placement means an implant is placed into the socket on the same day a tooth is extracted. Immediate loading means a provisional crown is fitted on the implant the same day it is placed. Both can be combined (extraction, placement and provisional all on day one) when conditions allow.

Am I a candidate for an immediate load implant?
Eligibility depends on bone density, bone quality, primary stability of the implant at placement, and your bite force. The CBCT scan and intra-operative measurements (insertion torque, stability quotient) tell us definitively. Anterior teeth, all-on-4 protocols, and patients with good bone are most often candidates. Heavy grinders and patients with poor bone quality may not be.
Will the same-day crown look like a real tooth?
It will look natural enough that no one will notice in conversation, the provisional is shaped, shaded and polished to blend with surrounding teeth. It is not the final, definitive restoration, however. The provisional is built for healing and aesthetics during osseointegration; the final crown delivered at 3 to 6 months has higher-grade materials and a more refined finish.
Can I eat normally with the immediate load provisional?
Not at first. The provisional is built to protect the implant during early integration, not to take heavy chewing load. We require a soft diet for the first 6 to 8 weeks, no biting into apples, no nuts, no chewy steak, no hard bread crusts. Most patients return to normal-soft eating at week 8 and full normal eating after the final restoration.
What is Straumann BLX?
Straumann BLX is an implant system specifically designed for immediate-load protocols. Its tapered design and aggressive thread pattern produce high primary stability at placement, the key prerequisite for safely loading the implant the same day. We use BLX in many of our single-tooth immediate-load cases.
How does All-on-4 immediate loading work?
All-on-4 was specifically designed for immediate loading. Four implants are placed in one arch, the rear two angled to engage strong cortical bone, and a provisional fixed bridge of 12 to 14 teeth is screwed onto them on the same day or the following day. The patient leaves the clinic with a fixed full-arch bridge in place, no removable denture period.
What if the implant fails to integrate?
If the implant fails to integrate within 6 months of placement, we re-place it at no additional cost. Documented failure rates for immediate-load implants are slightly higher than delayed-load (1 to 3 percentage points in most studies), but still in the high-90s percent range when patients are properly selected.
Is immediate loading better for international patients?
Often yes. Immediate loading lets international patients leave Vietnam with a fixed temporary crown or bridge already in place after a single trip, rather than wearing a removable temporary or having a visible gap during osseointegration. The final restoration is fitted on the second trip 3 to 6 months later.
Where is immediate load surgery performed?
At any of our six branches. Hospital-based delivery (Vinmec International Hospital Da Nang and Link General Hospital Da Lat) is recommended for All-on-4 immediate-load cases that take 3 to 5 hours and benefit from on-site sedation and emergency medical support.
How long does the implant last vs delayed loading?
Long-term survival is comparable. The systematic review evidence shows immediate-load implants survive in the 94 to 97 percent range at 10 years vs 95 to 98 percent for delayed-load, when patients are appropriately selected. The implant fixture itself, once integrated, has the same expected lifespan.
