Why your choice of implant system matters

When patients ask which implant brand will be used during a consultation, it tells me they have done their homework. That is exactly the right question. Vietnam's dental implant market has expanded rapidly, and with it the number of implant brands available. Walk through any dental supply conference in Ho Chi Minh City or Hanoi and you will encounter representatives from dozens of manufacturers. Availability does not equal quality.

Over the years we have treated patients who came to Picasso after experiencing failed implants placed at other clinics using brands we had never heard of. These systems often have attractive websites and notably low prices. Initially, everything appears fine on X-rays. Then, six months later, bone loss begins, inflammation follows, and sometimes the implant fails entirely. The cost of removing and replacing a failed implant is far higher than the saving made at the time of placement.

That depth of evidence matters when the stakes are high.

I turn to Nobel Biocare for the most demanding clinical situations. Limited bone volume where other systems would require extensive grafting before implant placement. Immediate loading protocols, where patients need teeth attached to implants on the same day as surgery. Compromised healing situations: patients with diabetes, smokers, patients on long-term steroid therapy, or those with autoimmune conditions that affect tissue repair. Full-arch reconstructions where multiple implants must integrate and function together with high precision. In every one of these situations, Nobel Biocare's TiUnite surface technology and the decades of documented clinical outcomes give me confidence I cannot get elsewhere.

Straumann: Swiss precision for the aesthetic zone

When replacing front teeth or working anywhere that appearance matters as much as function, Straumann is typically my first choice. Front teeth present unique challenges. Everyone sees them when you smile, talk, or laugh. The soft tissue architecture around an anterior implant, the way the gum contours around the crown, is as important as the crown material itself.

Straumann's Roxolid material, a titanium-zirconium alloy, allows for narrower implant diameters without compromising strength. This is crucial when working in confined spaces, when adjacent roots limit the available width, or when patients have naturally smaller jaw structures. The SLActive surface technology promotes faster bone cell attachment at the molecular level, which means faster integration and better soft tissue outcomes in situations where margins are precise. In my experience, the aesthetic predictability with Straumann in the anterior zone is consistently better than with any other system I have used.

Osstem: where quality meets value

Not every patient needs a premium Swiss or Swedish implant system. Osstem is a Korean implant brand that has genuinely impressed me over the past several years. For straightforward cases with good bone quality, healthy patients, and posterior tooth replacements, Osstem performs comparably to the premium European systems in every clinical metric I track.

Osstem has invested heavily in research and development, holds ISO certifications and CE marks, and their manufacturing precision consistently meets the standards required for long-term osseointegration. My patients do consistently well with these implants in posterior positions. I typically recommend Osstem for back teeth where function and longevity matter more than aesthetic subtlety. It is the choice that delivers excellent outcomes while keeping treatment within reach for a wider range of patients.

How I decide which system each patient needs

Five factors guide my recommendation at every consultation:

• Bone quality and quantity Healthy dense bone in a posterior site is often well served by Osstem. Compromised bone requiring maximum predictability points toward Nobel Biocare. Narrow ridges, particularly in the anterior zone, suit Straumann's narrow-diameter options.
• Tooth position Front teeth visible in your smile benefit most from Straumann's aesthetic tissue outcomes. Back molars and premolars are well served by Osstem. Complex full-arch cases default to Nobel Biocare for its immediate-loading protocols.
• Medical history and healing capacity Conditions affecting healing, including diabetes, immune suppression, and heavy tobacco use, push the recommendation toward Nobel Biocare, where the evidence base for compromised healing is deepest.
• Treatment goals and timeline Patients with immediate loading requirements (same-day teeth) need Nobel Biocare's validated protocols. Patients with adequate healing time have more options.
• Budget and value priorities I will not recommend a more expensive system when a quality mid-range option achieves equivalent results for a given case. Osstem in a straightforward posterior site is not a compromise; it is the clinically appropriate choice.

Why familiarity matters more than brand recognition

Here is an insight that may surprise you: the implant brand matters significantly less than your surgeon's depth of experience with that specific system. Our surgeons work with these three systems every week. They know precisely how Nobel implants behave during placement in different bone densities. They can feel the subtle tactile feedback when an Osstem implant reaches ideal primary stability. They know Straumann's tissue responses in the anterior zone in a way that only comes from repeated direct experience.

We would rather be true experts in three systems than merely competent with ten. When you deeply know a system, you can make real-time micro-adjustments during surgery that optimize success probability. You recognize edge cases the moment they arise, not after reviewing them later. You can troubleshoot immediately if primary stability is lower than planned. That deep familiarity is the variable that most influences outcome, more than the brand name on the implant body itself.
